How many pixels is one millimetre?

It depends on DPI: pixels = millimetres ÷ 25.4 × DPI. Keep fractional math internally and round in a way that preserves final physical size.

Can the calculator guarantee labels align?

No. It establishes geometry, while the printer, driver, feed and stock introduce tolerances. Use a 100% calibration print and enter measured offsets.
